dc.description.abstractAntimicrobial resistance (AMR) is concern to every human being worldwide, with the World newlineHealth Organization (WHO) identifying it among the top 10 threats to public health. AMR occurs when microorganisms such as bacteria, fungi, and viruses develop the ability to resist the effects of medications, particularly antibiotics that were once effective in treating infections. So for as current needs of clinical practice to rationalize antibiotic therapy and reduce newlinerecurrence of AMR, WHO has developed one software to tackle this problem. This is the newlinePharmacist led observational ambi-spective study which was carried out in 750 bedded tertiary care teaching hospital. Those patients satisfying inclusion criteria were included in the study and 700 patient data collected from different department of hospital. Collected data have been added into the WHONET software and analyzed for the different aspect. In our study, female newlinepatient are more in comparison to male patients and majority isolates were identified in the 18 65 yrs of age group followed by more than 65 yrs of age and least cases observed in below 12-18 yrs of age. The study collected and analyzed 700 culture samples from different clinical sites, which were categorized into eight broad groups: urine, blood, pus, sputum, wound, pleural newlinefluid, swab, and tissue in which most of bacterial species Urine (220), blood (181) and lease cases observed in Tissue (15). Escherichia coli - (67 %) was isolated mostly in urine samples, Staphylococcus aureus - (45 %) in blood and Acinetobacter sp. - (29 %) newlineobserved in respiratory samples. A total of 28 bacterial species were detected, out of which 20 were Gram-negative, representing 71.42 % of all isolates, while the remaining 28.57% were Gram-positive bacteria. Staphylococcus aureus and Escherichia coli being the most frequently isolated pathogens. Staphylococcus aureus, a gram-positive bacterium, accounted for 31.42% of all isolates, while Escherichia coli, a gram-negative bacterium, made up 23.28% of the newlineisolates.
