Evaluation of modified shock index as a predictor of mortality in polytrauma an observational study

Abstract

Introduction newlineTrauma related death is a major concern globally and with the implication of triage system at emergency department, the mortality levels reduced drastically. In most hospitals, triage is done by a medical expertise based on the patient s vital signs, age, levels of consciousness and presenting complaints. This study is designed to verify the association of a bed side tool named as Modified Shock Index in patients with polytrauma. newlineAim and Objective newlineAmong the polytrauma patients visiting the emergency department of a tertiary care center in Chennai, to determine whether the modified shock index (MSI) can be utilized for early detection of mortality in polytrauma patients and to compare the mortality prediction rate with other vital parameters. newlineMaterials and Methods newlineIt is an observational study, carried out in Emergency Department at Chettinad Hospital and Research Institute. After obtaining ethical committee approval, the study was conducted from February 2023 to January 2024, with a sample size of 50 patients. Patients were enrolled based on inclusion and exclusion criteria. Vitals of critically injured patients were recorded on arrival to emergency department. Thereafter, vital parameters were recorded every hour and MSI and SI were computed every sixth hour. Calculation done by MedCalc Software. newlineResults newlineThe current study revealed that the modified shock index (MSI), as opposed to heart rate, blood pressure, and the shock index (SI), is a better tool for polytrauma patients managed at emergency departments. It was also linked to a higher prediction of mortality in critically sick patients. newlineConclusion newlineThis study shows that, in contrast to the shock index and other vital indicators, the Modified shock index serves as a bedside marker that may be used to predict the outcome of emergency patients with polytrauma and is potentially helpful for triaging. newline newline
