newlineBACKGROUND
newlineSurgical extraction or a transalveolar extraction is a routinely performed oral
newlinesurgical procedure in which acute postoperative pain is a commonly
newlineexperienced symptom by the patient. The underlying severity of this pain can
newlinebe quite variable depending upon the type of surgical procedure. Management
newlineof pain after surgical tooth extractions has been a challenging part of dentistry.
newlineFor years researchers have sought the best analgesia after tooth extractions.
newlineTwo pharmacologic classes of drugs are used in the treatment of pain: the
newlinecentrally acting narcotic analgesics, such as codeine and oxycodone, and the
newlineperipherally acting analgesics, such as the nonsteroidal anti-inflammatory
newlinedrugs (NSAIDs). The choice of drug to be used as an analgesic is based on the
newlineduration of analgesic effect and minimal side effects. The NSAlDs are quite
newlineeffective against mild to moderate pain; however, not entirely free from adverse
newlineeffects. Gastrointestinal alterations like nausea, vomiting, ulcers are commonly
newlineassociated with all NSAIDs.Piroxicam is a nonsteroidal anti-inflammatory drug
newlineof the oxicam derivatives class used commonly to relieve the symptoms of
newlinepostoperative dental pain.
newlineIn this study we aim to use a dispersible piroxicam tablet locally(intra-socket) in
newlinecomparison with oral piroxicam to evaluate postoperative pain and swelling.

newlineTo compare the efficacy of intra-socket delivery of piroxicam with oral
newlinepiroxicam in postoperative pain management after surgical extraction of
newlinemandibular molars.
newlineMATERIALS AND METHODS
newlineHundred patients who had undergone transalveolar/ surgical extraction of
newlinemandibular molars were randomly divided into two groups: group A (oral
newlinepiroxicam 20mg), group B (intra-socket piroxicam 20mg). The dispersible
newlinepiroxicam tablet was applied to the extraction sockets and covered with
newlineresorbable gelatin sponges. Pain after extraction was evaluated using a visual
newlineanalog scale (VAS) at 6th and 24th hours postoperatively.
