The aim of the study is to evaluate the Siddha formulation-Thetran vidhai kudineer in the Management of Neerizhivu (Diabetes Mellitus- Type-2) by an open clinical trial. To conclude, the poly herbal Siddha formulation of Thetran vidhai kudineer can be used as a safe and effective drug for Neerizhivu (Diabetes mellitus-Type II). The derived results from chemical standardization of Thetran Vithai Kutinir Chooranam could serve as a ready reference for quality control assessment of the drug in future. HPTLC finger print profiles and quantification of gallic acid may be useful for the quality check of commercial drug in future. In conclusion, the present investigations demonstrates that at doses consumed in the traditional medicine of Thetran vidhai kudineer upto 3600mg/kg b.w as relatively safe, as it did not cause either mortality or significant drug treatment related effects on clinical signs or behavioral activity, biochemical, hormonal and histological changes etc., were observed in all the groups of rats that were survived during the experimental period. This study provides very important data on the toxicity profile of the Thetranvidhai kudineer that would be very useful for future studies in different animal models and large sample clinical study in Siddha system of medicine. The serum glucose, lipid profile, SGOT, SGPT and TNF-alpha levels shown to be decreased in TVK treated diabetic animals. Body weight, HDL, Insulin levels shown to be increased in TVK treated diabetic animals. The findings of the present investigation suggest that TVK has potential for its evaluation as an anti-diabetic agent against Fructose and STZ induced diabetes. Assessment of TVK for its underlying mechanism(s) will be an interesting topic and requires further study. An open clinical trial on Neerizhivu (Diabetes mellitus-Type II) gave encouraging results. The clinical features like polyuria (excessive urination), polyphagia (excessive hunger), polydypsia (excessive thirst) were reduced after treatment and quality of life was improved.
