| dc.description.abstract | Evaluation of Analgesic Effect of Panchtikta Ghrita as an Adjuvant Therapy in Patients of Fissurectomy newlineAbstract newlineBackground: Postoperative pain has always been a sensitive factor in patients of anorectal diseases. NSAIDs and other anti-inflammatory drugs are helpful but are associated with gastric and renal adverse effects. Acharya Sushruta has advised snehapana for pain management of sadyovrana. Application of this principle as pre and postoperative Snehapana on patients of fissurectomy can have an add-on analgesic effect. Objectives: To assess and compare severity of pain and dose of analgesia required for fissurectomy pain in controlled group and Panchatikta Ghrita intervention group. Methodology: Each group had 47 patients. In Experimental group Panchtikta Ghrita was given 3 days prior and 7 days post fissurectomy daily. In control group placebo drug was given similarly. Dose of Diclofenac was given in patients of both group as per need. Severity of postoperative pain was assessed on VAS. Results :Superiority observed in Interventional group was 72.34 % for Pain, 82.98 % for dose of analgesia required, 29.79 % for wound healing, 21.65 % for number of days required for complete healing and 31.88 % for Time off from work. Average dose of analgesia required and mean time off from work was 240.43 mg and 9.36 days in Panchtikta ghrita trial group compared to 426.60 mg and 13.74 days in control group. Ghrita being sneha dravya aids pacifying Vat dosha responsible for pain. Most of the other drugs in Panchtikta ghrita are having significant anti-inflammatory property which synergistically reduces severity of fissurectomy pain.
