Effectiveness of Transitional Care Nursing Package on Adaptive Health Behaviour and Health Related Quality of Life among Clients with Coronary Artery Disease

ABSTRACT newline Introduction: India is experiencing epidemiologic transition with sharp increase in Coronary Artery Disease (CAD). Improving patient s knowledge of their illness and awareness of behavioral changes helps to prevent a new event or readmission to hospital. Aims andobjectives: The study was conducted to evaluate the effectiveness of transitional care nursing package on adaptive health behaviour and Health related Quality of Life. Methodology: A quantitative approach with quasi experimental, pre-test and post-test design was used among 146 samples in study group and 144 samples in the control group by convenience sampling technique. Background variables, structured questionnaire on adaptive health behaviour and 36-Item Short Form Survey (SF-36) was used. Findings : Post test was done and the findings were in both groups majority of them were males, having BMI between 25 to 29 kg/m2 and WHR more than 0.95 (M) ,0.85 (F) . Majority 129 (88.4%) in study group and 131 (91%) in control group were having history of hypertension with hypercholestremia (Total cholestroland#8805;200 mg/dl) and were having habit of tobacco use. The study also found improvement in adaptive health behavior gain score (48.1%) and HRQOL (19.5%) in study group whereas in control group 0.4% and 1.32% only respectively. Conclusion: Hence the present study concluded that transitional care nursing package was highly effective in improving the adaptive health behavior level and the HRQOL of CAD clients. newline
