newlineDiabetes Mellitus is a major global health challenge, with India alone reporting over 77 million cases and a prevalence of 11.4% among adults. The rising burden is driven by rapid urbanization, increased life expectancy, obesity, sedentary lifestyle, and physical inactivity. Although insulin and oral hypoglycaemic agents offer significant advances, long-term use may lead to adverse effects, prompting the need for safer, holistic alternatives. In Ayurveda, Diabetes Mellitus correlates with Madhumeha, arising from Vand#257;tavand#7771;ddhi, Dhand#257;tukand#7779;aya, and faulty dietary and lifestyle habits, resulting in Kapha Meda dushti. Management emphasizes nidand#257;na parivarjana, dietary regulation, lifestyle modification, and Rasand#257;yana therapy.Methods:A randomized controlled clinical study was conducted on 100 patients of Madhumeha at D.Y. Patil Ayurvedic Hospital, Navi Mumbai. Participants were divided into two groups: Group A (Control,): Metformin 500 mg twice daily after meals Group B (Study,): Guduchi Swarasa 15 ml twice daily before meals with honey as anupand#257;naThe intervention continued for 90 days with follow-up every 15 days. Laboratory investigations were repeated at the end of the trial.
newlineResults: Excessive indulgence of Nidana of madhumeha Aasyasukha, Swapnasukha, Avyayama and Guru, Snigdha Ahara leads to Vitiation of Kapha and Meda and leads to disease.Madhumeha can be prevented through dietary and lifestyle factors. Nidana parivarjana is major component in treating disease, if we avoid Nidana Madhura, Amla, Guru, Snigdha Ahar in our daily diet we can arrest progress of disease and prevent Madhumeha.Prabhand#363;ta mutratand#257; was the most prevalent symptom. Group B showed a highly significant (Plt0.0001) reduction in fasting and postprandial blood sugar levels, improvement in clinical parameters(Plt0.0001) prabhoota avila mutrata, pipasaadhikya, karpada daha, hasta pada suptata, swedatipravritti.Conclusion:The study validates nidana parivarjan arrest progression of disease and the antidiabetic efficacy of Guduchi Swarasa in Madhumeha as a safe, cost-effective, and holistic therapy, it holds promise as an adjunct to conventional treatment.
